Dear Patient,

After 30 very enjoyable years, I will be closing my Plastic Surgery practice on December 31, 2016, to pursue other creative activities.

I am also pleased to announce that my associate, Dr. Aaron Kosins, will be taking over my practice. Dr. Kosins and I have worked together for the past 5 years and both share a passion for Rhinoplasty Surgery. During this time, we have discussed virtually every aspect of Rhinoplasty Surgery and operated together on numerous cases. I am confident that Dr. Kosins will provide the quality of care and surgical skill that you may require. If you do not wish to place yourself under Dr. Kosins' care, you can contact me directly until December 31, 2016. After that day, you can contact either the Rhinoplasty Society of America (www.rhinoplastysociety.org) or the American Society of Plastic Surgery (1-800-514-5058) for a surgeon who specializes in Rhinoplasty Surgery.

You will find on this website an authorization form to release a copy of your medical records. On receipt of this signed form, my office will forward a copy of your medical records to you or to the physician you designate. If you have not selected a new physician by December 31, 2016, you can obtain acute, critical, or emergency care by calling Dr. Kosins (1-949-721-0494) or the Emergency Room of your local hospital.

In closing, I have often said that the greatest privilege I have received in life is the trust that my patients have placed in me for transforming and enhancing both their physical and emotional well-being. It is a responsibility that I have taken very seriously and have dedicated my entire career to achieving for each individual patient.
